REPEAT PRESCRIPTIONS

As from 1St June 2008, we will no longer be able to take your repeat prescription requests over the telephone. Medication is becoming more complex and we hope by making these changes, fewer mistakes will occur.

YOU MAY ORDER YOUR PRESCRIPTION IN THE FOLLOWING WAYS

1. At the Surgery — You can bring your repeat order request slip into the Surgery marking the items required with a tick and place in the box provided in reception.
2. By Post — Post your repeat order request slip to us marking the items required with a tick, addressing your envelope to the Dispensary. If you wish for it to be returned by post, please enclose a stamped address envelope. Alternatively, we can send the signed prescription to the chemist at Waitrose, Eaton, or you can collect it from the Surgery.
3. Via the Chemist — Some local Pharmacies offer a prescription service, please contact them for details.


By E-Mail — We will not be able to process your request before 1st June 2008. Our e-mail address is cringleford.dispensary@nhs.net  (Please note we can only handle requests for repeat prescriptions via this service.) You will need to provide the following information: Full name, address and date of birth, together with the repeat medication you require including the dosage. An example of where you can find this information from on your prescription form is attached. Also please indicate if you wish us to send your signed prescription to the chemist in Waitrose, Eaton. It is vitally important that you provide us with a daytime telephone number, as we are unable to reply to your e-mails.
